Exception handling and escalation

Making an exception a decision someone can take in a minute instead of an investigation they postpone.

The problem

Automation moves the bottleneck to the exception queue. An exception typically arrives as a line on a list saying something did not match, and the person receiving it has to work out what, find the documents, and reconstruct the context — so it is postponed, and the queue ages. The exceptions that matter most are usually the ones that look hardest to start.
